Online Therapy · Cape Town & South Africa
Evidence-based online therapy for adults across South Africa, delivered securely by HPCSA-registered clinical psychologists.
Online therapy from Cape Town
Barnardt & Fleming is a Cape Town–based clinical psychology practice offering online therapy for adults across South Africa. Because we work over secure video, there's no waiting room and no commute, you meet your psychologist from wherever you feel most at ease.
Whether you're in the City Bowl, the Southern Suburbs, or hundreds of kilometres away in another province, you receive the same considered, evidence-based care and a genuine therapeutic relationship — the foundation of meaningful change.
How we work with adults
The same therapy, delivered differently.
Online therapy is psychotherapy conducted through secure video consultation. The principles, ethics and therapeutic process remain the same as face-to-face therapy. For many people, the primary difference is simply where the conversation takes place.
Sessions are tailored to your individual needs and delivered using evidence-based approaches appropriate to your concerns. Depending on your goals, therapy may incorporate Cognitive Behavioural Therapy (CBT), Metacognitive Therapy (MCT), Psychodynamic Therapy, EMDR, or an integrative approach. Therapy comes to you, allowing you to engage from a familiar environment while receiving the same standard of professional psychological care.
The Research
Emerging research has shown that psychotherapy delivered by secure video consultation can be as effective as face-to-face therapy for many common psychological difficulties when clinically appropriate. Evidence supports online treatment for conditions including anxiety disorders, depression, post-traumatic stress disorder (PTSD), stress-related difficulties and adjustment problems. Professional organisations, including the American Psychological Association (APA), increasingly recognise telepsychology as an effective way of delivering psychological care when clinically appropriate.
Not every person or every clinical situation is suited to online therapy. As with any psychological treatment, your psychologist will consider your individual needs, current circumstances and treatment goals when deciding whether online therapy is the most appropriate option.
More than simply convenient.
For many adults, online therapy has become more than an alternative to attending a clinic or office — it is increasingly becoming a preferred way of accessing psychological care. People often choose online therapy because:
Skip the traffic and the time off.
Sessions fit more easily around work, family and other commitments.
Receive psychological support regardless of where you live in South Africa.
Attend from a familiar, private environment — no waiting room or travel required.
Private video sessions over a secure connection, handled in line with POPIA.
The right format depends on the individual. For many people, online therapy offers both flexibility and clinically effective care.
The Therapeutic Relationship
One of the strongest predictors of successful therapy is the quality of the therapeutic relationship. Research consistently shows that a collaborative, trusting relationship between psychologist and client contributes significantly to positive outcomes across different therapeutic approaches.
Although sessions take place through a screen, many people quickly stop noticing the technology and experience the conversation much as they would in person. The focus remains on understanding your experiences, working towards meaningful change, and developing a relationship that feels safe, respectful and genuine.
The Experience
Online therapy is often quieter and more natural than people expect. You'll meet with your psychologist from a private space where you feel comfortable. Sessions follow the same structure as traditional therapy, with time to explore what has brought you to therapy, reflect on patterns and experiences, and work towards practical and emotional change.
Many people appreciate having time after the session to remain in their own environment rather than immediately returning to work or travelling home. This can create space for reflection before continuing with the rest of the day.
Beginning Therapy
The first session is an opportunity to understand what has brought you to therapy and what you hope to gain from the process. Your psychologist will ask about your current concerns, relevant personal history, relationships, health and previous experiences of therapy where appropriate. Together, you'll begin developing an understanding of your difficulties and discuss possible treatment approaches. There is no expectation to share more than feels manageable. Therapy develops over time, at a pace that feels appropriate for you.
What you'll need
Online therapy requires very little preparation. Ideally you'll have:
A quiet, private space where you feel able to speak openly
A reliable internet connection
A computer or tablet with a camera and microphone
A comfortable place to sit without interruptions
A glass of water and tissues nearby so you won't need to step away
Headphones can improve privacy and sound quality but are not essential
When it may be a good fit
Online therapy can be particularly helpful for people who:
Prefer the privacy of meeting from home
Have demanding work schedules or family responsibilities
Live outside major cities and want access to specialist psychological care
Travel regularly for work within South Africa
Find travelling difficult because of health concerns, disability or anxiety
Find it easier to open up and reflect from a familiar environment, making therapy feel more natural than attending an office.
The suitability of online therapy is always considered individually rather than assumed.
Important Considerations
Online therapy is not the right setting for every situation. In-person care may be more appropriate for:
Acute crises or active suicidality, where immediate in-person or emergency support is safer
Certain dissociative presentations, where being physically present with a psychologist may better support safety and grounding
Situations requiring physical or in-person clinical assessment
Some presentations of severe mental illness where a higher level of in-person support is needed
Suitability is always assessed individually. If online therapy is unlikely to meet your needs, we'll discuss appropriate alternatives, including referral for in-person care where necessary. Your safety and wellbeing will always guide these decisions.
